CO Mental Health Therapist

Job Description

The Cornerstone Clinic at Helping Up Mission is seeking a Mental Health Therapist to add to our multidisciplinary treatment team. Cornerstone Clinic is within the Johns Hopkins University School of Medicine, Psychiatry Department. As a Mental Health Therapist, the clinician will work independently and within a team environment utilizing multiple treatment modalities to provide both individual and group therapy to persons who are dual diagnosed with mental health and substance use disorders. Additionally, the clinician in this position will work with physicians, mid-levels, primary care providers, and other specialists to promote qualitative and efficient use of health care services. This position is a flexible hybrid position, offering in-person and telehealth sessions on pre-selected days

 

Specific Duties/Responsibilities


Clinical

  • Establishes initial and on-going therapeutic relationship with Cornerstone clients in need of mental health services.
  • Completes intake sessions with individuals referred for mental health treatment.
  • Communicates results of assessment as medically necessary to staff involved in clients’ care.
  • Coordinates care with other Cornerstone clinic treatment team members.
  • Participates in development of an initial plan of care to address prioritized needs of client.
  • Provides ongoing services as prescribed on plan of care.
  • Provides mental health, substance use and healthy behaviors therapy in an individual and group setting.
  • Makes referrals for additional services as needed.
  • Assists clients in securing external behavioral health resources as needed.
  • Engages with community resources to ensure client is receiving necessary services.
  • Re-evaluates plan of care with client and other Cornerstone clinical treatment providers to assist clients in meeting identified treatment goals.
  • Develops, recommends and initiates corrective action through participation in, team coordination consultation and contributions to the clients’ care plan.
  • Identifies and acts upon impending changes in clients’ condition.
  • Provides outreach to clients if he/she disengages from treatment.
  • Provides crisis intervention as needed.
  • Regularly handles multiple member cases involving a variety of providers, situations, deadlines and multiple priorities.
  • Assist program leadership in the implementation of quality improvement and professional development projects
  • Performs other clinical duties as assigned.


Documentation (in accordance with Cornerstone clinic policy and all regulatory boards, (i.e., CARF, COMAR, and local agencies)

  • Completes initial evaluations using established procedures.
  • Complies with reporting requirements of various state and local agencies.
  • Completes plan of care using established procedures.
  • Delivers ongoing care as defined in the plan of care using established policy.
  • Writes progress notes and treatment plans.
  • Updates plan of care using established procedures.
  • Crosstrain and provide coverage of colleagues across different primary care sites.


Special Knowledge, Skills, & Abilities

  • Knowledge of mental illness, substance use, health behaviors, and community resources.
  • Competent in multiple treatment modalities including Motivational interviewing, Cognitive Behavioral Therapy, Behavioral activation and must either have skills or willingness to learn office-based substance use disorder interventions and Health Behavior interventions.
  • Analytical skills to resolve highly complex problems requiring the application of scientific or technical principles, theories and concepts and in-depth, and experienced-based cross-functional knowledge.
  • Ability to provide professional and empathic support.
  • High level of interpersonal skills.
  • Excellent written and verbal communication skills.
  • Self-motivated and able to work both independently and in a multidisciplinary treatment team environment.
  • Open to ongoing supervision in specific therapeutic modalities.
  • Organized and detail oriented.
  • Concentration and constant attention to accuracy and detail for extended periods of time.
  • Flexibility and adaptability.
  • Able to enter data into electronic information systems.


Equipment, Machine, or Tool Requirements

  • Computers, general office and communication equipment.
  • Regularly utilizes EPIC and other information systems to perform daily duties and document care.


Physical Requirements

  • Work is generally sedentary in nature; however, some standing, stooping, and bending are required.


Minimum Qualifications
  • Master’s Degree in Counseling, Social Work, or related discipline.
  • Two years’ experience, preferably working and delivering independent outpatient care to the identified population in an outpatient setting.
  • Current Social Work or Counseling Licensure in the State of Maryland- LCSW-C or LCPC required and must maintain license throughout duration of employment.
